Changing Your Family Doctor

Dear visitors, if you have changed your residence or simply wish to register with another family doctor within MedFamily, below you will find the necessary steps.

How to change your family doctor:

The following steps are based on the Regulation on the registration of a person with a family doctor from a medical institution providing primary healthcare within the compulsory health insurance system.

Terminology:

IMS AMP – medical institution providing primary healthcare under the compulsory health insurance system;
CNAM – National Health Insurance Company;
AT – Territorial Agency of CNAM;
AT representative – employee of the Territorial Agency of CNAM responsible for the IMS AMP.

Important to know:

If the chosen family doctor serves an area different from the patient’s place of residence, the patient must ensure that the doctor and/or nurse can travel to their home whenever needed.
The change of family doctor and IMS AMP can be made at any time of the year, but not earlier than 6 months after the last change or initial registration (except when the person has changed residence).
The official basis for confirmation of registration is the Application for registration with a family doctor (Form No. 1-33/c) registered in the information system.

If a person requests to change the family doctor and IMS AMP without changing their place of residence, or if the person has moved to a new address, the registration will be carried out as follows:

the application, completed by the applicant (sections 1–7) and approved by the family doctor (section 8), must be submitted in person to the AT CNAM representative (Vasile Lupu St. 18);
the AT CNAM representative verifies the accuracy of the application within 5 working days, updates the information system, fills in section 9, and applies the official stamp;
the AT CNAM representative then sends the application to the responsible person within MedFamily and informs both the applicant and MedFamily of the changes made within 10 calendar days.
